Members of the Sharp Community Medical Group (SCMG) HMO, in which case you will need a valid referral from your SCMG Primary Care Physician before scheduling an appointment.
Patients whose referring Primary Care Physician is a Sharp Community Medical Group (SCMG) Doctor. In this case, patients may have either Medicare, Tricare, or Contracted PPOs. However, please note that we are not contracted with Health Net PPO, United PPO, or Cigna PPO.
Patients who have had a history of Melanoma, regardless of their primary care physician.
First Responders and active members of the Military.
